Workshop Registration Forms and Fees
Calendar workshop fees include: Refreshments, training materials, Certificate of Attendance and lunch for full day workshops.
Group discounts apply to organisations with multiple representation at a single calendar workshop. More than five registrations from the same organisation to the same workshop attract a 5% discount.

Programs for each workshop will be posted to health care organisations in the designated State approximately six weeks prior to the workshop date.

To apply for any of the listed workshop programs please complete a registration form and forward with payment to: ACHS Education and Support Service, 5 Macarthur Street

Closing Date for Registration
We would appreciate receiving your registration seven days before the workshop, otherwise it cannot be guaranteed that training material and a certificate will be available at the workshop.
Confirmation
Places in workshops cannot be confirmed unless registration and payment are received prior to the closing date. A letter confirming your place in a workshop will be sent to you. It will provide you with the address of the workshop venue, parking/transport details and other relevant information. Cancellation Policy
The ACHS reserves the right to cancel any workshop. Full registration will be refunded if a workshop is cancelled or oversubscribed. Refunds, less $44 administration fee, will be granted when at least three working days notification of inability to attend a workshop has been given. Alternatively, a nominated replacement may attend.
