The goal of the Root Cause Analysis (RCA) process is to find out what happened, why it happened, and to determine what can be done to prevent it from happening again.

This RCA training is being conducted as two, 3-hour virtual interactive sessions over two days. It will take you through all the steps to conduct an RCA for severe harm and frequent clinical incidents. The training will introduce you to systems thinking and focus on improving and redesigning systems and processes to prevent harm to future patients. A key aspect to be explored will be the relationship between the system of care and individual human performance to improve the effectiveness of recommendations.

Due to the popularity of this course and in response to feedback, we have now changed the course to two X 3-hour sessions and we are limiting the number of participants to 100.

Be able to develop recommendations which can be tested using plan, do, study, act cycles
Be able to formulate an implementation plan to ensure recommendations drive continuous improvement in patient quality and safety
