Background Information on Root Cause Analysis in Australia

In Australian public and private hospitals it is a requirement that serious adverse events undergo a clinical investigation. In 2005 the Australian health system adapted the Veteran’s Administration USA Root Cause Analysis methodology. In addition all States and Territories have legal privilege legislation in relation to the requirement to conduct an Root Cause Analysis and provide a report to the Department/ Ministry of Health and report the findings of the investigation to the family.

The ACHS as one of its offerings through the Improvement Academy is providing RCA training workshops, in Australia and to international members.

RCA One Day Course Learning Objectives:

  • Understand legislative requirements for RCA
  • Understand the purpose of root cause analysis (RCA)
  • Understand when to undertake an RCA based on understanding of special and common cause variation
  • Have a practical understanding of the steps of the RCA process
  • Develop skills in conducting staff interviews to identify the sources of process failure
  • Be able to develop recommendations which can be implemented to improve care processes
  • Have developed sufficient knowledge in order to actively participate and contribute to an RCA team, if lead by an experienced practitioner.

Who should attend?

Health care professionals including: clinical managers and medical heads of departments, surgical and procedural team leaders, directors of patient safety, patient safety officers and frontline clinicians who provide care and services to patients.

Prior learning

Clinician or Clinician Manager.

RCA Trainers

The ACHS has the capacity, through its membership and networks, to harness the skills and knowledge of senior managers and clinicians across the health system in all states and territories. The ACHS has drawn upon its ongoing relationship with these individuals to bring together a faculty with the relevant skills and experience to deliver this training. 

Assessment

Participants will be assessed during the workshop in terms of their engagement and participation in the practical skills sets.

CPD Point

Participation in this program will qualify participants for continuing professional development hours, as required by their respective National Board.

ACHS certificate

Will be issued upon full completion of the course.
