The goal of the Root Cause Analysis (RCA) process is to find out what happened, why it happened, and to determine what can be done to prevent it from happening again.This RCA training is being conducted as two, 3-hour virtual interactive sessions over two days. It will take you through all the steps to conduct an RCA for severe harm and frequent clinical incidents. The training will introduce you to systems thinking and focus on improving and redesigning systems and processes to prevent harm to future patients. A key aspect to be explored will be the relationship between the system of care and individual human performance to improve the effectiveness of recommendations.
Due to the popularity of this course and in response to feedback, we have now changed the course to two X 3-hour sessions and we are limiting the number of participants to 100.
Please note that registrations for all ACHS Improvement Academy public training events are managed by Eventbrite which accepts payment with credit or debit card. We are unable to issue invoices outside of this system. Registrants will receive an automated tax invoice/receipt once they have finalised their payment. This can be used for reimbursement purposes.

The ACHS reserves the right to cancel any program. Full registration will be refunded if a program is cancelled or oversubscribed.

Cancellations received in writing by ACHS more than 7 days prior to the event will be accepted and all fees refunded less 10% (ex GST) of the total cost of the training. Cancellations received within 7 days of the event will not be refunded, however, transfer of your registration to another person is acceptable.

- Understand how Root Cause Analysis links with Clinical Incident Management and Quality Improvement as per the Clinical Governance Framework and Standard 1 NSQHS second edition
- Understand the policy and legislative requirements of undertaking a Root Cause Analysis
- Have a practical understanding of the steps of the RCA process
- Develop skills in conducting staff interviews to identify the sources of clinical process failure
- Be able to develop recommendations which can be tested using plan, do, study, act cycles
- Be able to formulate an implementation plan to ensure recommendations drive continuous improvement in patient quality and safety
