What is RCA? 

Root Cause Analysis (RCA) in healthcare, is one of several methodologies which involve a systems focused approach to identifying the root causes of adverse events and uncovering the underlying factors that contributed to these events. The training equips healthcare professionals with the knowledge and skills to undertake the steps required for an effective serious adverse incident review and to make recommendations to prevent similar incidents from occurring in the future. 

How is the course run? 

Training is conducted as two, 3-hour virtual interactive sessions over consecutive days. Participants practice undertaking an RCA using a pre-allocated case study. Past attendees have found our RCA training to be engaging, practical and interactive.

Key outcomes 

  • Understand how Root Cause Analysis links with Clinical Incident Management and Quality Improvement as per the Clinical Governance Framework and Standard 1 NSQHS second edition 

  • Understand the policy and legislative requirements of undertaking a Root Cause Analysis 

  • Have a practical understanding of the steps of the RCA process 

  • Develop skills in conducting staff interviews to identify the sources of clinical process failure 

  • Be able to develop recommendations which can be tested using plan, do, study, act cycles 

  • Be able to formulate an implementation plan to ensure recommendations drive continuous improvement in patient quality and safety
