Supporting an Aged Care Facility in Melbourne with Incident Investigation Training

The Improvement Academy delivered a Root Cause Analysis (RCA) training workshop at Royal Freemasons Aged Care Service.

Improvement Academy Director, Associate Professor Bernie Harrison delivered a custom one-day Root Cause Analysis (RCA) workshop at the Royal Freemasons Aged Care Service, held at the Coppin Centre in Melbourne. The session brought together 25 senior managers from seven residential aged care facilities across Victoria, all focused on strengthening their incident investigation skills. 

RCA is a structured method used to understand the underlying factors that contribute to incidents and adverse events. In the aged care sector, where the safety and wellbeing of older Australians is paramount, RCA plays a critical role. It not only helps organisations identify what went wrong, but also why it happened, enabling teams to implement effective, lasting improvements in care delivery and risk management.

The workshop guided participants to understand the Serious Incident Response Scheme (SIRS), have a practical understanding of the steps of the RCA process and how to write a comprehensive RCA report.  

The participants were highly engaged, demonstrating a strong grasp of both the theory and practical application of RCA. They actively contributed to the discussions about the challenges faced by aged care services and managing safety for their clients. They presented as a cohesive and professional team of individuals who collectively demonstrated commitment and compassion to their clients.
