The ACHS Improvement Academy held a custom training session on Root Cause Analysis at Life Care in Adelaide, South Australia.
Life Care supports older South Australians with holistic home care, nursing, allied health and spiritual care services. The session brought together clinical and case managers who provide comprehensive support to older Australians and help them maintain their health and well-being at home. Clinical and case managers perform a variety of roles including clinical assessments, care planning and coordination of support services.

The training session run by Improvement Academy Director, Bernie Harrison, focused on how RCA incident investigation can be conducted effectively, with specific Aged Care examples. Life Care takes on a person-centred approach to care that focuses on fostering independence, dignity, and quality of life. Embracing structured methodologies like RCA can help strengthen this approach, helping teams to respond to incidents with insight, compassion, and accountability. The session also highlighted how taking a systemic approach to incident investigation helps uncover underlying causes and leads to more sustainable improvements in care.
